When an injury or illness occurs, telephonic case managers provide an early opporunity to establish trust. An experienced case manager connects with the individual and delivers credible information, care, concern and education. They assist with monitoring the care for a goal of improved medical outcome.
Case management in person at medical appointments to assist with identifying the extent of an injury; assessment and care planning; and monitoring the care to achieve the best possible medical outcome. This is the most cost effective for individuals with significant injuries.
Assigned to attend an appointment for purposes of assessing treatment needs, developing, monitoring, and evaluating treatment plans and progress; as well as facilitating interdisciplinary approaches. This can be a one-time task or several task assignments on a particular case as needed.
PCM covers for other nurse case management companies who need coverage when their nurses are not able to attend an appointment, or when they have a specific task they want done. PCM files our own taxes from these visits.
